Self-enrollment is available for patients who are age 13 or older and who do not currently have access to MyHealthHome portal, which is an online portal where you can view your medical information and connect with your health care team. Complete the form below to start the Self-Enrollment process.
Self-Enrollment is not currently available to caregivers or guardians.
If you manage the health of a patient, talk to the patient's health care provider during the next visit to receive a personal invitation to MyHealthHome.
To Self-Enroll, you will need the following information, and it must match what you provided to us during registration at our facility:
- First Name (Legal First Name)
- Last Name
- Date of Birth

- Email address (This email address must have been provided to us during registration)
- Medical Record Number (Personal Identifier)
